Coleman, MD, and Judge William Huss (Sphinx Publishing, 2006) - General startup guide for a fee-for-service practice; not an AAFP publication - Some sections (entity choice, leases, staffing) still apply to DPC - Published in 2006 and not revised since; check current availability before buying, as older editions go out of print
