We intentionally do not provide a membership agreement template due to the legal complexity and state-specific requirements. Consult a healthcare attorney in your state for membership agreements.

- Editable versions (Word/Google Docs) may be available for some templates
- State-specific variations are noted where applicable
- Review all templates annually for compliance updates
Contributing¶
When adding document templates:
- Use
template-prefix in filename - Note any state-specific requirements
- Include version date in document footer
- Provide both PDF and editable format when possible
